ノカルジア感染による肉芽腫性皮膚炎の犬の1例

要　　　　　約 　 右肘部皮下に発生した腫瘤を主訴に5歳齢，雄のボクサー犬が来院した．針生検により非腫瘍性腫瘤と判断して腫瘤を外科的に切除した．病理組織学的検索によりノカルジア感染による化膿性肉芽腫性皮膚炎と診断され，長期の抗菌薬による内科療法がなされた．腫瘤の切除により生じた広範な皮膚欠損部は開放創として頻回に洗浄するとともに，創の清浄化と肉芽形成促進を目的としてキトサンを，創の保護と上皮形成促進を目的としてキチンを使用した．その結果，術創は順調に縮小して術後3カ月で閉鎖し，現在18カ月経過しているが再発は認められていない． ―キーワード：犬，ノカルジア症，化膿性肉芽腫性皮膚炎． -------------------日獣会誌　57，113〜115（2004） Canine Pyogranulomatous Dermatitis Caused by Nocardia-subspecies Infection Hiroshi UENO＊†, Tsuyoshi KADOSAWA, Kenji OCHIAI, Yukiko TOMIOKA, Masahiro OKUMURA and Toru FUJINAGA ＊Laboratory of Veterinary Surgery, Department of Veterinary Clinical Sciences, Graduate School of Veterinary Medicine, Hokkaido University Kita 18-jo Nishi 9-chome, Kita-ku, Sapporo, 060-0818, Japan SUMMARY 　Cytological examination showed a large subcutaneous mass on the right lateral elbow of a 5-year-old boxer dog to be pyogranulomatous dermatitis. The entire mass was excised. Because histopathological examination revealed fungal hyphae of Nocardia, we diagnosed pyogranulomatous dermatitis caused by Nocardia subspecies. Long-term adjunctive antibiotic therapy was applied. The large open wound was washed frequently and treated with chitosan, which has incarnant effects. In addition, the wound was covered with chitin-coating dressing as an epithelialization accelerator. On the 60th day, the wound had been reduced about one-tenth from its original size and on the 74th day one-fiftieth. On the 88th day, it was fully closed.
