僧帽弁閉鎖不全症の高齢犬に外科と化学療法
を併用した原発性肺腺癌の1例

要　　　　　約 　発咳，そして左前肢挙上を主訴として17歳，雄のシーズー犬が来院した．患犬は僧帽弁閉鎖不全症の病歴があった．X線検査では左肺前葉部に単一で孤立性の不透過性集塊陰影像，超音波検査では実質性の腫瘤が描出された．同所見より肺腫瘍と判断し，左肺前葉摘出と肺門リンパ節の郭清手術を行った．この腫瘍は病理組織学的に，肺腺癌と診断された．また，組織学的にリンパ節への転移が認められた．この腫瘍は形態や発生部位から原発性肺腺癌と考えられた．本症例に対してはカルボプラチンおよび硫酸ビンブラスチンを併用した化学療法を実施した．初回の投与後に軽度の白血球減少が認められたため，投与予定を7日延期した．その後はプロトコールに従って投与した．術後3カ月目から進行性の後躯麻痺が出現し，MRI検査により，第12胸椎椎体部に腫瘍の転移を疑わせる所見が得られた．プレドニゾロンの投与により後躯麻痺は一時的に改善が見られ，ふらつきながらも歩行が可能となった．呼吸器症状などの悪化はなく経過は良好であったが，体力の低下により術後6カ月半で死亡した．高齢でステージもかなり進行した肺腺癌の症例に対し，積極的に治療を行った結果QOLを十分に改善したものと推測された．また，併用化学療法の有効性が示唆された． ―キーワード：犬，肺癌，化学療法 --------------------日獣会誌　55，679〜682（2002） Combination Chemotherapy on Primary Lung Adenocarcinoma in an Old Dog Arao YAMAOKA†, Shizu HASHIMOTO, Shuji FURUKAWA, Sanae SAEGUSA and Hozumi YAMAMURA Kitagawa Animal Hospital, 1-39-1 Minami-tokiwadai, Itabashi-ku 174-0072, Japan SUMMARY 　A 17-year-old Shih Tzu with a history of mitral valve regurgitation was admitted to the hospital because of coughing and habitual lifting of a left front leg. Radiography revealed a solitary, markedly radio-dense nodule in the left cranial lobe of the lung. Total lobectomy of the tumorous left cranial lobe and removal of the hilar lymph nodes were performed. The diagnosis of pulmonary adenocarcinoma was made on the basis of histopathological findings. Metastasis to the regional lymph nodes was confirmed. Combination chemotherapy with carboplatin and vinblastin sulfate was initiated 10 days after surgery. The mild leucopenia observed a week later necessitated delaying second treatment for seven days. Thereafter the rest of the treatment protocol was carried out without complication. Paresis of the hind limb was observed 90 days after surgery. MRI revealed possible tumor metastasis on the 12th thoracic vertebral body. The dog's condition improved temporarily with administration to prednisolone, but later paresis of the hind limb advanced. The dogﾕs condition deteriorated and it died 196 days after surgery. This case suggests that aggressive treatment can enable an old dog with relatively advanced pulmonary adenocarcinoma to live with a reasonable quality of life.
