犬の水頭症に対するジメチルスルホキシドの1適用例

要     約

 重篤な神経症状を急激に発現したシー・ズー,雄,140日齢に,神経症状の軽減を図る目的で,脳圧亢進と神経細胞の2次的傷害を抑制するジメチルスルホキシド(Dimethyl Sulfoxide; DMSO)を投与した.患畜は後日水頭症と確定診断され,第50病日に斃死するまでの経過中,DMSOの反復投与を主体とした内科的治療により発作はおおむね抑えられ,意識状態は良好で,在宅生活を維持した.発作による体力消耗が防止されたことも,QOL維持,余命延長に大幅に寄与したと考える.DMSOは神経症状の原因確定を待つ間や頭蓋内出血の可能性が否定されない症例の維持的療法に有用であると考える.

Case Study of Dimethyl Sulfoxide Administration
to a Hydrocephalic Dog

SUMMARY    

Dimethyl Sulfoxide (DMSO), which is known to decrease intracranial pressure and to inhibit secondary damage to nervous tissue effectively, was administered intravenously to a dog for the purpose of medically improving central nervous system (CNS) symptoms. The patient, a male 140-day-old Shih Tzu dog was attacked by sudden, severe seizures caused by hydrocephalus. Primarily medical treatment with intermittent DMSO administration (l g/kg BW, i.v. drip) almost entirely inhibited the seizures till the patient died 50 days after the onset. Though it gradually weakened, the patient remained well conscious and continued living with its family till its last morning. By retarding strength decline caused by the seizures, the therapy contributed to preserving the quality of life while prolonging it. We recognize that DMSO is useful in supportive therapy during the period when the causes of CNS symptoms are being determined and against the cases where the possibility of intracranial hemorrhage cannot be ruled out.
